If you would like to reserve the use of the Clinical Performance Center for an existing event, please complete the form below.

All requests must be affiliated with University of Chicago Medicine, Continuing Medical Education, Pritzker School of Medicine, research or training. Requests will be evaluated based on stipulations provided and must be approved by the Medical Director of the CPC or simulation space.

Please submit reservation requests at least two months in advance.
